It is located beneath the window on the southeastern wall of the Surb Astvatsatsin (Holy Mother of God) Church. The rectangular tombstone features a four-line inscription enclosed in a double frame at the center of its southern face: «Այս է տապան լուսահ|օգի Տ[է]ր Մելքիսէթին | Տ[է]ր Ավաքեանց եւ հայր | էր Տ[է]ր Սարգիսին Թ ՌՅԻԸ (1879)» (This is the tomb of the luminous soul (the late) of Tēr Melkʻisētʻ1 of Tēr Avakʻeants and father of Tēr Sargis, year 1879).
To the right and left of the inscription, beneath the semicircular apses, simple blossoming crosses are carved on semicircular pedestals. It is notable that the last letters of the second and third lines of the inscription are carved onto the frame, while the last two letters of the fourth line are under the left arm of the horizontal cross carving on the right. The southern face of the tombstone is bordered on both sides and at the top by a wide ornamental belt.
